1. Are your services confidential?

All personal information discussed in therapy is completely confidential and cannot be released to a third party without your written consent, except under special circumstances when the law requires disclosure. As a therapist, specific instances when I would be required to break confidentiality include: if you are a danger to yourself or others, participate in elder or child abuse or if there is a court-ordered subpoena.

2. Do you accept insurance?

My services are often covered by mental health insurance policies but I do not personally submit the paperwork to the company. It is best for you to find out about your insurance benefits as they differ from provider to provider. I will provide you with a monthly bill that has the information required for your reimbursement. It is your responsibility to explore what paperwork you need to receive reimbursement. I will do anything I can to aid you in getting assistance in the cost of therapy.

3. What do you charge for a session?

Currently my fee for a 50-minute session is $220. I accept credit cards. If you prefer to pay by check, please have your checks made out to WADE DAVIS before your session to allow maximum use of your time. I do have some lower fee slots available if you need assistance.

4. What is your cancelation policy?

In order for therapy to be effective a client must commit to the process. My style allows for clients to eventually move to every other week schedules if needed. Each client "owns" his/her time slot and are responsible for the fee if I do not get a 24-hour cancelation notice.

5. What is your policy regarding minors?

When a patient who is a minor is in individual therapy, the parent or guardian has the right to ask for information about the minor's therapy. I will need to have a confidential relationship in order for trust to build with a minor client, but will involve the parent or guardian in session to keep all up-to-date.
